Can't find the link, but apparently it blew up her oxygen supply and they don't know if she'll pull through.... wouldn't have thought an e-cig would get hot enough.... headlines like this won't help the cause

a glowing coil perhaps, if she was sucking the pure oxygen through a dried out wick. I don't know the critical temperatures, but I think pure oxygen is quite easy to ignite.

Oxygen from those tanks only needs a spark to ignite. It burns out pretty fast with no fuel to keep it going. I've seen people with those oxygen tanks up their noses and they are smoking, if they accidentally melt the plastic tube they could ignite the oxygen.

Pure oxygen will not ignite at any pressure. There must be a reactant, ie, a fuel which can then be oxidised. Even as a liquid combinant in rockets, oxygen must have a 'fuel' such as hydrogen , etc.
