Where to punch the hole ?
Put a carto into the base and with a felt tip pen, or pencil or scratch with something sharp etc, make a mark on the cartomiser just above the top level of the base of the tank unit - so the resulting hole lies above the base allowing the juice to get into it once the tank is on and filled up.
Edit: Basically, the hole needs to be as low down as possible without being blocked off by the tank base once you insert the cartomiser.....ie: not halfway up or the juice won`t get in once the level drops.

I punch mine as low as sensibly possible. Two holes, not too big mind you, just enough to break the metal (you can feel it go....) I did two BIG old holes and it just flooded. Rather oddly, I find the pre-punched ones are too small.....so much for "laser drilled" accuracy.

Used to use a junior hacksaw (or Dremel-a-like) and slit them instead - just make sure thee remove any swarf or debris after doing so (and watch thy knuckles)
