Hi OP,
I work in mental health and have experience of various units nationally - the stance seems to vary quite a lot.
There are some units where cigarette use has been banned *completely*, there are some where it is permitted. There are also some where it has been 'banned' but a blind eye is most definitely turned for various reasons :-)
There are some very real discussions currently about e-cigs.... early days, involved in some of these myself, and watching with interest....
Positions seem rarely concrete here (except in medium/higher secure services due to e cigs being an ignition source and potential risks here) - partly, I suspect, as a result of some interesting ongoing legal challenges about use of real cigarettes...
