My friend has tried Ego type batteries back in the day, with little success. I can't even remember what atties he was using because this must be over 4 years ago...
Anyway, fast forward, he is facing major heart surgery, wants to reduce his 40 fag a day habit. I bought him an iStick and some 24mg juices from JJ and LiQuid.
What I need now is a decent clearomiser, preferably not a bottom fill one, that won't give that 'burnt' taste on the iStick, and that will work with the Ego/510 adapter.
Needs to be simple..

I agree with Perpetua. I have used the Iclear 16 since the came out never had a problem with any of them.

Just a heads up. As a Heavy (no equivalent to Cigarettes, but by the end I was still seeing 1 smouldering as I Rolled the next!) ex-Rollie Smoker I DID use iClear 16 for a couple of days, but quickly found I ran low/out of juice so experienced Dry Hits. Moving to iClear 30 (3ml ish) meant I could judge better when I was running low and equally wasn't panicking if I left all my juice at home.

I loathed the iclear 16 and indeed all top coiled atomisers, dry hits are all too common unless you keep swooshing the juice and they are very much two years ago technology now - the draw is very, very tight comparatively and airflow control is really something very difficult to give up once you try it.
The istick is perfectly accompanied by a nautilus mini - yes it is a bottom fill device but thats not really a huge burden and providing the coils are primed when you first use them they appreciate a higher wattage and so will have no issues with the stick's tendency to be a tad over powered

The full size Nautilus looks fine on an iStick and is by far the easiest clearo I've used when it comes to filling/cleaning/coil changing. As good a vape as you'll get without rebuilding/buying a sub ohm tank too.

I clear 16's are ideal if the person is familiar with their quirks if you want a less hassle clearo then the nautilus is a good choice or a subtank mini with 1.2ohm coils and give your friend the aaec address so he can get any help he needs when he needs take some of the weight from you and we have insomniacs .

I'd try the aerotank v2 much better than the iclear 16 or 30

There's little wrong with the iclesr16 in my view. Personally I can't use them but that's because I lung inhale and need airflow. And they are way two tight for me now. But let's not forget this is a newbie recommendation and if I goback 12 months ago they were spot on.
They're also one of the easiest to fill which is what was requested in the OP.
A nautilus is a step up but not as easy to fill for some.

Maybe try an evod 2 or evod glass. Pretty good vape but they don't like anything above say 9 watts. Easy to fill and change coils (even my Mrs can do it)

I'm pretty new to caping, only been on about a month or so and I have to say the Nautilus Mini has been by far the best for me. Easy to fill, clean and change coils. Just my 2 pennies hope he gets on ok. As you said, agreeing is half the battle. I was dubious to start with but now I've started there's no going back to stinkies for me
