My current favourite setup is my svd, with a 3.5ml vivi nova, 2.1ohm coil at 4v with a 12mg caramel toffee. The 30s, protank and k100 are just sitting there but im still not happy :\

I have a couple of kayfun clones, wicked with microcoil & cotton. There's a lot wrong with them (they are clones), but the actual vape seems to me to be significantly better than the novas, iclears, protanks Nautilus & aga-t that I have. It's the first thing I've had which has me reducing the nic level because I don't need the throat hit, and I shall reduce the flavour strength in some of my mixes too.
So I think vape quality is about having a decent chimney, and an adjustable air flow directly opposite it, with the coil between them.
You might get some improvement with a dual coil, with two air holes, one under each, as in the aqua, but I'm not sure it will make that much difference. And of course there will be people who regard the power I put through mine as grossly inadequate and want a mech mod, but so far I don't see the benefit.
The rest is getting a design which will deliver that. The difficulty is stopping the juice going down the air hole. The current solution is to create a low pressure area above the juice in the tank, which seems to me to be less than ideal, but I don't have an alternative to suggest.
Then you have to have a decent size of tank - ideally enough for a day's vaping - and for me, a glass tank (the aqua, incidentally, has neither of these things) and a hassle free refilling system (which my kayfun clones don't have). Of course the deck needs to be stainless steel, and the whole thing shouldn't leak, whichever way up it is.
After that, I think, it is just aesthetics - at least until someone develops a cigarette-sized battery that will last all day.

I'm unsure what my ideal set-up is yet. I have tried a lot of different gear (although nowhere near as much as some).
In my hand just now is my V4 Vamo in 18350 mode, with a Russian 91% clone, running Minty The Toff. It's okay. Right next to it is my SID with an Aspire BDC running Red Astaire. It is so freaking close to my perfect vape it's unreal.
I am currently on the lookout for a Semovar V3. I'm pretty happy with the Aspire BDC, so a slightly better mod (for muchos more coin) may improve it a little, but I'm not really positive on that.

At the moment for me it is not the equipment that I am having problems with, it's finding the ideal all day vaping liquid I'm struggling with. I have excellent results with the Kayfuns, Diver, iClear 30 and even the Aga T3 and TD. The closest to a flavour that I can vape all day is the custard types, but even these get a bit sickly after a while. I have tried mixing my own favourites to keep the cost down, but like the OP has said I'm still chasing the holy grail of vapes, but will probably never find it.

I didn't do juice because I don't see me ever sticking to one juice any more than I'd settle for one flavour of food.

I am, perhaps for the first time and after buying far too much gear, super happy with my set up. I love my kayfun clones with a cotton/microcoil set up - they're not perfect for the reason giles mentions, but they are significantly better than the clearos that I've tried in flavour and maintenance. Maybe I've been lucky, but I've not had a single problem with flooding or gurgling, and the flavour, throat hit and taste are great. My iclear30s are also reasonable, and make suitable backups. I have lovely juice from t-juice that I can mix myself, a selection of mods that do the job, and I feel really well set up - until the next big thing hits of course
